Fatal error at Review Steps 3 of OCS

When I attempted to review the submission to provide feedback (step 3 of review) a new window popped open with a fatal error message listed below.

I followed the same path and found that all the files under the comment directory existed so not sure what this error message means. Any thoughts on what be happening here ?
